If so, we want to hear about your experience, knowledge, and comfort with the Human Rights Complaint process! We are looking to gather information for all ages and we really need help with feedback from children and individuals in the community. Please consider filling out the survey below

For your reference, some examples of what is covered by DBHDS include:

  • ID/DD Waiver services through a Community Service Board (CSB)
  • Any services through a CSB
  • State facilities
  • Assisted Living Facilities and group homes operated by a CSB
  • Other housing programs operated by a CSB
